Please note: This order form lists our standard
shipping charges - NOT the real time shipping charges which are automatically
calculated on this site. (Our real time shipping rates take weight,
rate and zone information into account for five different domestic shipping methods.
We will never be able to put this on a paper order form!)
If you prefer
to use a hard copy order form, you are welcome to calculate your real time shipping
charges on this site and use them on the order form. You do not have to log in
or create an account to do so. Simply add the desired items to your cart, provide your
zip code and the system will list the rates for all available shipping options.
You may also telephone or email us and we will calculate the real time
shipping prices for you if you do not wish to use the standard pricing.
